Booking Terms
By booking an appointment with DW Aesthetic Clinic, you acknowledge and agree to the following terms and conditions.
Booking Fee
A non-refundable $50 booking fee is required to secure your appointment. Subject to these terms, the booking fee will be applied toward the cost of your consultation or treatment on the day of your appointment.
Late Arrival
If you arrive late, the clinic may shorten, modify or reschedule your appointment at the practitioner’s discretion. Where insufficient time remains for the appointment to proceed safely or appropriately, the appointment may be cancelled by the clinic and treated as a late cancellation, in which case the booking fee will be forfeited.
Cancellation and Rescheduling
A minimum of 24 hours’ notice is required to cancel or reschedule an appointment. Any request to reschedule within 24 hours of the scheduled appointment time must be made by calling the clinic directly. Online cancellation or rescheduling may be unavailable within the applicable blackout period. If less than 24 hours’ notice is given, or if you do not attend your appointment, the $50 booking fee will be forfeited.
Consultation and Clinical Assessment
All appointments are subject to clinical assessment, practitioner availability, and compliance with applicable legal, regulatory and professional obligations. Booking an appointment does not guarantee that any particular treatment, procedure, product or prescription will be recommended, offered or provided. The $50 booking fee covers the practitioner’s time, consultation, assessment and clinical advice, whether or not treatment proceeds on the day.
Practitioner Discretion
The clinic and treating practitioner reserve the right to decline, defer, or modify treatment where, in the practitioner’s professional judgment, treatment is not clinically appropriate, safe, lawful, indicated, or in the client’s best interests.
Health Information
You must provide complete, accurate and current information regarding your medical history, medications, allergies, prior procedures, and any other information relevant to your care. Failure to provide accurate or complete information may result in the appointment being postponed or treatment being declined, and the booking fee may be forfeited.
Clinical Photography
Clinical photography may be required as part of your health record, assessment, treatment planning, monitoring of progress, and medico-legal documentation. If required photography cannot be obtained, the practitioner may be unable to proceed with treatment.
Fees and Payment
Payment for consultations and treatments is due at the time services are provided unless otherwise agreed in writing. Any card surcharge will be disclosed and will not exceed the clinic’s actual cost of processing that payment method.
No Guaranteed Outcome
Individual responses and outcomes vary. The clinic does not guarantee any particular outcome, duration of result, or suitability for any treatment.
Refunds, Credits and Packages
Booking fees are non-refundable except where required by law. Change of mind does not entitle a client to a refund. Any package, prepayment or account credit is subject to the clinic’s separate written terms and is not redeemable for cash unless required by law.
Clinic Safety and Conduct
The clinic reserves the right to refuse or discontinue services where a client behaves in an abusive, aggressive, threatening, unsafe or otherwise inappropriate manner, or where proceeding would pose a risk to any person or to the safe operation of the clinic.
Future Bookings
Where a client repeatedly late-cancels, fails to attend, or otherwise breaches these terms, the clinic may require full prepayment for future appointments or may refuse future bookings.
Acceptance
By paying a booking fee, making a booking, or attending an appointment, you confirm that you have read, understood and accepted these terms.
